Zusammenfassung
Die physikalische Medizin wird vielfach nach ihren Therapiemitteln definiert, die als „physikalisch“ charakterisiert werden können (Wärme, Kälte, Bewegung, Druck, Elektrizität u. a.). Auf der anderen Seite werden die physikalischen Therapien auch als solche Therapien beschrieben, bei denen die Auslösung körperlicher Reaktionen im Sinne von kurzzeitigen oder langfristigen Adaptationen im Vordergrund steht (Reiz-Reaktions-Prinzip). Dies ist insofern von Bedeutung, als nicht nur die direkten Effekte der Therapien, sondern vor allem auch die indirekten physiologischen Reaktionen auf sie als wesentliches Wirkprinzip betrachtet werden müssen. In der Therapie von akuten und chronischen Schmerzen können physikalische Therapien daher einerseits dadurch eine Bedeutung haben, dass sie direkte analgetische Eigenschaften besitzen (z. B. Kältetherapie und einige Formen der Elektrotherapie) oder andererseits dadurch, dass sie Funktionsstörungen, die an der Entstehung von Schmerzen beteiligt sind, beeinflussen (z. B. manuelle Therapie, medizinische Trainingstherapie). Letzteres schließt auch die Modulation von Schmerzschwellen (z. B. aerobes Training) auf Ebene der peripheren und zentralen Schmerzregulation mit ein.
