Polyartikuläre Verlaufsformen der juvenilen idiopathischen Arthritis

Zusammenfassung

Bei der juvenilen idiopathischen Arthritis kann eine Polyarthritis, der Befall von zumindest 5 Gelenken, bereits initial bestehen oder sich im Verlauf aus einer Oligoarthritis oder anderen JIA-Kategorien entwickeln. Einerseits ist die Prognose bei einem polyartikulären Verlauf eher zweifelhaft und insbesondere bei der rheumafaktorpositiven Polyarthritis wird nur selten eine stabile medikamentenfreie Remission erreicht, anderseits stehen für die Polyarthritis die meisten geprüft wirksamen und zugelassen Medikamente zur Verfügung. Hier ist als konventionelles Basistherapeutikum insbesondere Methotrexat zu nennen und dann die Gruppe der Biologika und hier v. a. die Inhibitoren der Zytokine TNF-α und Interleukin-6. Diese Therapieoptionen ermöglichen moderne Therapiekonzepte nach dem Treat-To-Target-Prinzip und damit ein frühes Erreichen einer Remission und die Vermeidung von Krankheitsschäden bei nicht allen, doch bei der großen Mehrheit der Patienten.
