Zusammenfassung

Unter den Fehlbildungen stellt die Duodenalatresie des Neugeborenen einen akuten Notfall dar, der durch die radiologische Untersuchung schnell diagnostiziert werden kann. Membranöse oder fibröse Duodenalstenosen machen sich oft erst mit Beginn der Beikostfütterung bemerkbar. Die häufigste entzündiche Erkrankung des Magens bei Kindern ist die durch Helicobacter pylori (H.p.) hervorgerufene Gastritis; Magen- und besonders Duodenalulzera als Komplikationen sind bei Kindern und Jugendlichen selten. Nichtinfektiöse Gastritis und Magenulzera können durch Medikamente (nichtsteroidale Antiphlogistika), galligen Reflux oder schwere Stresssituationen hervorgerufen werden. Alle übrigen Gastritisformen (lymphozytäre, kollagene oder atrophische Gastritis) sind selten, ebenso wie benigne und maligne Tumoren. Die infantile Pylorushypertrophie ist eine genetisch determinierte Erkrankung, die aber durch Umwelteinflüsse, z. B. die Gabe von Makrolidantibiotika begünstigt wird. Eine beschleunigte Magenentleerung verursacht ein Dumping-Syndrom und entsteht oft postoperativ, während die Gastroparese durch verschiedene Grundkrankheiten verursacht wird. Unter den funktionellen Störungen ist das zyklische Erbrechen mit einer Häufigkeit von 2–3 % bei Kindern eine diagnostische und therapeutische Herausforderung.
