Bei psychischen Belastungen ist zwischen ursächlichen Faktoren und Folgezuständen der Adipositasidealiter zu unterscheiden, was aber i.d.R. nicht möglich ist. Während in den 1950er Jahren die Sichtweise verbreitet war, Adipositas stelle eine Störung der Persönlichkeit dar, die auf einen Mangel in der von Freud postulierten oralen Phase zurückgeht oder Ausdruck einer psychischen Abwehr gegenüber negativen Gefühlen ist (z. B. Angst und Depression), interpretiert die moderne Adipositasforschung seelische Aspekte vorherrschend als abhängige Variablen. So zeigen katamnestische Untersuchungen, dass eine Gewichtsreduktion i.d.R. mit einer Besserung psychischer Symptome, insbesondere von Angst und Depressivität, verbunden ist.
