Zusammenfassung

Viele Formen der assistierten Reproduktion sind für die Behandlung der Infertilität des Mannes wirksam. Inseminationsbehandlungen sind primär geeignet bei gestörter Abgabe der Samenzellen. Die Wirksamkeit einer Insemination ist allerdings bei verminderter Spermienqualität nicht erwiesen. Hingegen ist heute die intrazytoplasmatische Spermieninjektion (ICSI) bei den unterschiedlichsten Beeinträchtigungen der männlichen Fertilität Therapie der ersten Wahl. ICSI kann auch mit Samenzellen erfolgreich durchgeführt werden, die mittels Biopsie aus dem Hoden gewonnen wurden. Beim vollständigen Fehlen der Spermatogenese bleibt die Samenspende die letzte Option. Für den Erfolg der assistierten Reproduktion ist das Vorhandensein einer reifen Eizelle unabdingbar. Die hormonelle Stimulation der Ovarien ermöglicht die Gewinnung von mehreren Eizellen, so dass in der Folge mehrere Pronukleus-Stadien oder befruchtete Eizellen entstehen können. Die Kryokonservierung ermöglicht nicht nur die Übertragung von weniger Embryonen pro Therapieversuch, sondern auch im Laufe der Zeit die Entstehung mehrerer Schwangerschaften mit nur einer ovariellen Follikelpunktion.
