Zusammenfassung
Für das neurologische Outcome der Kinder hat das Temperaturmanagement eine entscheidende Bedeutung. Moderate (28–32 °C) und schwere Hypothermien (<28 °C) führen zu Herzkreislaufversagen und Herzrhythmusstörungen. Patienten, die einen Atem-Kreislaufstillstand unter Hypothermie erleiden, haben eine bessere Prognose als normotherme Patienten. Im Anschluss an eine Reanimation soll bei Kindern jenseits des Neugeborenenalters eine strikte Normothermie eingehalten werden. Patienten mit ausgeprägter Hyperthermie müssen aufgrund des hohen Risikos für eine Rhabdomyolyse und ein Multiorganversagen innerhalb von 30 min unter 39 °C gekühlt werden. Die Ausdehnung thermischer Verletzungen wird im Kindesalter nach der Handflächenregel eingeschätzt, wobei nur Areale ab Grad 2 einberechnet werden. Bei der Verbrennung bzw. Verbrühung stehen die Analgesie und eine angepasste Flüssigkeitszufuhr an oberster Stelle.
