Zusammenfassung
Unter dem Begriff Aortenaneurysma versteht man eine permanente, umschriebene Erweiterung der Aorta auf einen mindestens 50 %igen Zuwachs des Durchmessers im Vergleich zum erwarteten normalen Durchmesser der Aorta. Die Häufigkeit von thorakalen Aortenaneurysmen beträgt zirka 5–10 pro 100.000 Patienten pro Jahr mit einem Häufigkeitsgipfel in der 6. und 7. Lebensdekade. Als Ursache liegt meist eine Wandschwäche durch Degeneration der Arterienwand auf dem Boden einer Arteriosklerose vor. Treten Aortenaneurysmen bereits im 2.–5. Lebensjahrzehnt auf, ist häufig eine genetische Grunderkrankung verantwortlich. Thorakale Aortenaneurysmen sind zunächst völlig asymptomatisch und werden oft als Zufallsbefund entdeckt. Die am meisten gefürchtete Komplikation ist eine Aortenruptur oder eine akute Aortendissektion. Das primäre Ziel ist es, eine elektive Operation anzustreben, um das Risiko einer Aortendissektion oder Ruptur durch Wiederherstellung eines normalen Aortendurchmessers zu vermeiden. Dabei wird das Aortenaneurysma komplett resezieren und durch die Implantation einer Gefäßprothese ersetzen.
