Abstract
The changing framework of oncological treatment with shorter periods of inpatient treatment, a shift to more outpatient care with more palliative therapies and shorter hospital stays necessitates early and correct identification of cancer patients suffering from distress and psychiatric comorbidity in order to ensure adequate psychosocial support and therapy. For clinical practice valid and practicable routine screening instruments as the HADS or the distress thermometer are available. They make it possible to screen reliably for highly distressed patients but screening represents only the first step. In a second step supplementary diagnostic evaluation of patients who are potentially in need of psycho-oncological care is required. The authors discuss characteristics of short screening instruments, report on experiences and results in transferring evidence-based screening procedures into routine care and give recommendations for clinical practice. The application of systematic screening procedures for emotional distress shows great promise for enhancing psycho-oncological care of cancer patients.
Zusammenfassung
Unter veränderten Rahmenbedingungen onkologischer Behandlung ist die frühzeitige, zutreffende Identifizierung hoch belasteter Tumorpatienten erforderlich, um eine adäquate psychoonkologische Behandlung zu gewährleisten. Für die klinische Routine sind valide und praktikable Screening-Instrumente verfügbar. Mit diesen können hoch belastete Patienten zuverlässig ermittelt werden, ‘überschwellige’, potenziell behandlungsbedürftige Patienten bedürfen ergänzend einer diagnostischen Abklärung. Die Autoren diskutieren Eigenschaften kurzer Screening-Instrumente, berichten über Erfahrungen und Ergebnisse bei der Umsetzung von Screening in der Routineversorgung und leiten daraus Empfehlungen für die Praxis ab. Der Einsatz systematischer Screening-Verfahren verspricht eine verbesserte psychoonkologische Betreuung von Krebspatienten.
