Zusammenfassung

Es wird zunächst gezeigt, dass bei angenähert normalem Eiweissgehalt und normaler Konfiguration der Vorderkammer, (d.h. wenn keine Verbindung mit dem Suprachorioidalraum oder mit der Subconjunctiva existiert) der in die Vorderkammer eintretende Flüssigkeitsstrom durch den Schlemm'schen Kanal abgeführt wird. Der Durchfluss ist dann durch die Gleichunq A = Pa/W definiert. Darin bedeutet A = das Minutenvolumen, Pa = den Abflussdruck und W = den Widerstand zwischen Vorderkammer und laminären Venen.

Es lässt sich zeigen, dass der Abflussdruck, d.h. der Druck, der den Abfluss aus der Vorderkammer beherrscht, unter dieser Bedingung der Unterschied zwischen Augendruck und Druck in den laminären Venen ist. Es werden dann die Methoden beschrieben, um den Druck in den laminären Venen und im Schlemm'schen Kanal zu messen. Der Druck in den laminären Venen beträgt 9,7 ± 2,34 mm Hg, der Druck im Schlemm'schen Kanal ist ca 1 1/2 mm höher. Beim einfachen Glaukom ist der Druck in den laminären Venen und im Schlemm'schen Kanal keineswegs höher, eher sogar niedriger als bei Normalen, trotzdem der Augendruck erhöht ist.
Der normale Abflussdruck (Tension-Druck in den laminären Venen) beträgt 5–7,5 mm. Bei primären chronischen Glaukomen ist er stark erhöht. Der Druckabfall erfolgt bei den bisher untersuchten Fällen von einfachem Glaukom zwischen Vorderkammer und Schlemm'schem Kanal.

Es wird eine Methode beschrieben, um beim Menschen das Minutenvolumen der Vorderkammer zu ermitteln. Das normale Minutenvolumen beträgt: 2,2 ± 0,37 mm3.
Bei den bisher untersuchten Fällen von „deinfachen” Glaukomen ohne kongestive Komponenten war das Minutenvolumen etwas subnormal (1,7 mm3). Bei chronisch „kongestiven” Glaukomen wurde bei gleichbleibend hohem Druck das Minutenvolumen sehr häufig vergrössert gefunden.

Die statistisch gesicherte Verminderung des mittleren Minutenvolumens bei einfachem Glaukom erklärt, warum man bei dieser Glaukomform vor allem ein negatives Ascher'sches Glasstabphänomen findet.

Der Widerstand ergibt sich als Quotient aus Abflussdruck und Minutenvolumen. Die normale Widerstandszahl war bei 24 normalen 3,0 ± 0,79; aus dem Gesamtmaterial berechnet sich die mittlere Widerstandszahl zu 3,2, davon entfallen 3/4 − 4/5 auf den Übergang aus der Vorderkammer in den Schlemm'schen Kanal und l/3 − l/5 auf dem Uebergang vom Schlemm'schen Kanal zu den laminären Venen. Der Widerstand ist sowohl beim „einfachen” als auch beim „kongestiven” chronischen Glaukom erhöht, bei jenem sogar meist mehr als bei diesem. Trotzdem ergibt die gonioskopische Untersuchung in der Phase der Drucksteigerung beim „einfachen” Glaukom einen offenen, beim „kongestiven” Glaukom einen weithin verschlossenen Kammerwinkel.
Beim „einfachen” Glaukom wurde der Widerstand zwischen Schlemm'schem Kanal und laminären Venen bisher normal gefunden; der erhöhte Widerstand sitzt also in diesen Fällen im Trabeculum corneosclerale, zwischen Vorderkammer und Schlemm'schem Kanal.

Im Anschluss an verschiedene Glaukomoperationen sahen wir Sekretionshemmung auftreten.

Für die Ansicht von Thomassen, die Drucksteigerung beim Glaukom werde durch Rückstauung des Flüssigkeitsstromes im Schlemm'schen Kanal bewirkt, haben wir bei primären chronischen Glaukomen keine Anhaltspunkte gefunden. Es wird gezeigt, dass die Befunde, die Thomassen an den Ciliarvenen erhoben hat, ohne Heranziehung besonderer pathologischer Mechanismen der Ciliarkörpercirculation bei Glaukomatösen erklärt werden können.

If the protein-content of the aqueous and the anatomical configuration of the anterior chamber are approximately normal (i.e. there is no connection between the anterior chamber and the suprachoroid or subconjunctival lymphatic spaces) then the fluid current leaves the anterior chamber by the canal of Schlemm. The rate of flow is then defined by the equation A = Pa/W. A is the rate of flow per minute, Pa is the outflow pressure and W the resistance between anterior chamber and the episcleral laminar veins.

The out-flow pressure i.e. the pressure governing the out-flow of the anterior chamber is then the difference between ocular tension and pressure in the laminar veins. Methods are described to measure the pressure in the laminar veins and in Schlemm's canal. The pressure in the laminar veins was found to be 9,7 ± 2,34 mm Hg.
The pressure in Schlemm's canal is ca 1 1/2 mm higher than in the laminar veins. These pressure-values are in cases of „simple” glaucoma with high ocular tension not higher than in normal cases. The normal out.-flow pressure (ocular tension minus pressure in the laminar veins) amounts to 5–7,5 mm. It is much higher in cases of primary chronic glaucoma. In cases of „simple” glaucoma the fall in this increased pressure is between the anterior chamber and Schlemm's canal (in the cases till now examined).

A method is described to measure the rate of flow of the anterior chamber in living human eyes. It amounts to 2,2 ± 0,37 mm3/min. In cases of simple glaucoma without congestive signs hitherto examined the rate of flow was found to be slightly sub-normal (1,7 mm3/min). In cases of chronic „congestive” glaucoma with an elevated tension, unchanged during the time of the examination, the rate of flow was increased.

The statistically significant diminution of the rate of flow in simple glaucoma explains why Aschers glassrod phen. is so often found negative in these cases.

The resistance is the quotient of out-flow pressure and rate of flow. The normal value of this figure is about 3,0. Approximately 3/4 of this value represent the resistance of the corneo-scleral trabeculum and 1/4 resistance of the outflow through the vessels out of Schlemm's canal. In cases of chronic „simple” and „congestive” glaucoma the resistance is elevated, in the former rather more than in the latter. Nevertheless gonioscopic examination shows an open angle in „simple” glaucoma and an almost or totally closed angle in „congestive” glaucoma during the phase of increased tension.
In the cases of „simple” glaucoma hitherto examined the resistance between Schlemm's canal and the laminar veins was found to be normal. Therefore the increased resistance is situated in the corneo-scleral trabeculum.

After different antiglaucomatous operations we saw a phase of diminished secretion.

Thomassen brought forward a hypothesis that the elevated tension in simple glaucoma is due to a hindering of the outflow from Schlemm's canal by an increased pressure in the episcleral veins. We could not find any proof for this assertion.
It can be shown that Thomassen's findings on the pressure in the ciliary veins in glaucoma can be understood without any assumption of special pathological mechanisms of the circulation of the blood through the ciliary body.
