Zusammenfassung
Parallel zu der zunehmenden Einnahme von gerinnungshemmenden Medikamenten wie Thrombozytenfunktionshemmer, Vitamin-K-Antagonisten und direkten oralen Antikoagulanzien steigt auch der Anteil der intrazerebralen Blutungen (ICB) bei Patienten, deren Gerinnungssystem beeinträchtigt ist. Gefürchtet ist hier vor allem eine unkontrollierte Hämatomexpansion mit potenziell fatalem Ausgang. Das Gerinnungsmanagement in der Akutphase einer intrazerebralen Blutung zur Vermeidung einer Hämatomexpansion ist daher ein weiteres essenzielles Therapieziel. Anhand der Ergebnisse von PATCH, INCH, REVERSE-AD und ANNEXA-4 wird der aktuelle Studienstand zusammengefasst, und im Kontext internationaler Leitlinien wird ein Fazit für die klinische Praxis gezogen. Zusätzlich werden Nutzen und Risiko einer Thromboseprophylaxe nach ICB anhand einer Metaanalyse von Paciaroni et al. diskutiert.
