Zusammenfassung

Patienten, die sich einer revaskularisierenden Operation im Gefäßsystem unterziehen müssen, benötigen eine langfristige antithrombotische Therapie. Gesichert ist eine Reduktion kardiovaskulärer Ereignisse für diese Patienten. Die aktuelle Studienlage empfiehlt in der medikamentösen Therapie nach lokaler Thrombendarteriektomie und nach alloplastischen Bypässen der aortalen und supragenualen Region ASS 100 mg/d oder Clopidogrel 75 mg/d. Nach Anlage kniegelenksüberschreitender alloplastischer Bypässe wird die Kombination von ASS 100 mg/d und Clopidogrel 75 mg/d empfohlen, für die Dauer einer dualen TFH gibt es jedoch keine gesicherte Evidenz. Dem gegenüber profitieren Patienten nach autologem Venenbypass von einer oralen Antikoagulation mit Vitamin K Antagonisten (Ziel-INR 2–3). Dabei muss das Blutungsrisiko regelmäßig reevaluiert werden, bei Kontraindikationen hingegen empfiehlt sich die Gabe von ASS 100 mg/d. Die Daten der aktuellen COMPASS- und VOYGER-PAD Studien implementieren eine niedrig dosierte orale Antikoagulation in Kombination mit ASS bei Patienten mit nicht/ mässig erhöhtem Blutungsrisiko nach infrainguinalen Revaskularisationen.
