Zusammenfassung
Beim Thema Wachkoma können wir eine traumatische und eine nichttraumatische Ursachengruppe unterscheiden. Das Verhältnis traumatisch zu nichttraumatisch liegt bei etwa 20 zu 80, wobei die traumatische Gruppe eher abnimmt und die nichttraumatische Gruppe aufgrund der verbesserten intensivmedizinischen Möglichkeiten zunimmt. Durch die Entwicklung der Intensivmedizin überleben viele Menschen und werden vielleicht auch ganz wiederhergestellt. Aber es überleben auch mehr mit bleibenden schwersten Behinderungen. Die Entwicklung des Wachkomas verläuft bei beiden Gruppen ähnlich. Aufgrund der immer vorhandenen Hirnschwellung kommt es zu einer Schädigung des Mittelhirns – es entsteht ein Mittelhirnsyndrom, das die Symptome des Wachkomas, insbesondere die Bewusstseinsstörung und die klassischen motorischen Schablonen schon vorwegnimmt. Kann dieser Zustand nicht erfolgreich behandelt werden, entstehen irreversible Schäden, und es entwickelt sich das klinische Bild des Wachkomas. Über die Häufigkeit, also die Inzidenz und Prävalenz des Wachkomas, gibt es über Jahrzehnte hinweg stark unterschiedliche Angaben. Das gibt Hinweise auf die Grundeinstellung zu dieser Patientengruppe, aber auch auf die Betreuungsqualität.
