Zusammenfassung
Die Therapie kardialer Arrhythmien hat sich in den letzten Jahrzehnten deutlich gewandelt. Mit zunehmender Verbesserung elektrischer Verfahren wie der Implantation eines elektrischen Defibrillators/Kardioverters (ICD) ist die Bedeutung pharmakologischer Maßnahmen rückläufig. Diese Entwicklung wurde durch die Erkenntnis gefördert, dass die meisten Antiarrhythmika selbst proarrhythmogen sein können und darüber hinaus negativ-inotrope Wirkungen besitzen, die unter bestimmten Bedingungen zu erhöhter Mortalität bei behandelten Patienten führen können. War die Pharmakotherapie früher die wesentliche Behandlungsmöglichkeit bei Arrhythmien, stellt sie heute in den meisten Fällen eher eine unterstützende Therapie im Rahmen anderer Maßnahmen dar.
