Zusammenfassung
Offene Verletzungen, Gefäßverletzungen oder Mangled Extremities treten in der Regel nach einem hochenergetischen Trauma auf. Diese Verletzungen stellen im Rahmen der Versorgung schwerverletzter Patienten eine besondere Herausforderung dar. Nach initialer Ruhigstellung und steriler Abdeckung der Wunden sind eine rasche und umfassende Anamnese, klinische Untersuchung und radiologische Diagnostik für eine adäquate Einschätzung und eine hierauf aufbauende Therapieentscheidung ausschlaggebend. Beim Vorliegen ausgeprägter Kombinationsverletzungen mit Beteiligung von Weichteilen und knöchernen sowie neurovaskulären Strukturen müssen neben den lokalen Faktoren zusätzlich der Allgemeinzustand und vorliegende Begleitverletzungen bei der Therapieentscheidung bezüglich des Erhalts der Extremität berücksichtigt werden.
