Zusammenfassung

Die operative Behandlung großer Prostatavolumina stellt eine Herausforderung in der Therapie des benignen Prostatasyndroms (BPS) dar. Die offen-chirurgisch, retropubisch oder transvesikal durchgeführte Prostatadenomenukleation (OPE) sind die häufigsten und weltweit am weitesten verbreiteten Techniken für große Prostataadenome. Die OPE stellt in den aktuellen Leitlinien der EAU, AUA und DGU bei Prostatavolumina > 80 ml trotz Einsatz minimal-invasiver transurethraler Enukleationsverfahren den Therapiestandard dar. Exzellente funktionelle (Langzeit-)Ergebnisse werden bei der OPE jedoch mit einer nicht zeitgemäßen perioperativen Morbidität erkauft. Obwohl die OPE in westlichen Ländern durch minimal-invasivere Methoden verdrängt wird, stellt die OPE in weniger entwickelten Ländern die wichtigste Therapieoption bei großen Prostatavolumina dar.
