Zusammenfassung
Läsionen des Plexus brachialis kommen überwiegend bei jungen Männern im Rahmen von Motorradunfällen vor. Die gesundheitlichen, aber auch psychosozialen Folgen der Verletzung sind für die Betroffenen von enormer Tragweite. Hinzu kommt, dass sich die Behandlung von Plexusläsionen über einen langen Zeitraum erstreckt. In diesem Kapitel werden im ersten Abschnitt die Plexusläsionen des Erwachsenen und im zweiten Abschnitt die geburtstraumatischen Plexusläsionen im Säuglingsalter beschrieben. Nach einer kurzen Einführung in die Anatomie des Plexus brachialis wird die klinische Präsentation verschiedener Plexusverletzungen und ihre Klassifikation erläutert. Neben wichtigen Aspekten der klinischen Untersuchung, wird die notwendige apparative Diagnostik vorgestellt. Operationsindikation und -timing werden aufgezeigt. Es wird ein ausführlicher Einblick in mögliche Therapiemaßnahmen gewährt. Um einen kompletten Überblick über die Versorgung von Plexusläsionen zu schaffen, werden die Nachbehandlung und auch die Therapiemöglichkeiten von Deafferenzierungsschmerzen thematisiert. Der zweite Abschnitt über geburtstraumatische Plexusläsionen ist vom Aufbau identisch. Geburtstraumatische Plexusläsionen unterscheiden sich jedoch unter anderem in Bezug auf den Traumamechanismus, die Prognose, die Indikationsstellung und das Timing von den Plexusläsionen im Erwachsenenalter, sodass sie in einem gesondertem Abschnitt besprochen werden.
