Zusammenfassung
In diesem Kapitel soll dargestellt werden, in welche Richtungen sich die Plexuschirurgie in den letzten 10 Jahren entwickelt hat. Diese Darstellung beruht auf Erfahrungen, die an eigenem Krankengut gewonnen werden konnten, auf neuen Erkenntnissen aus dem Schrifttum und nicht zuletzt auf der Analyse von Patienten, die auswärts wegen einer Plexus-brachialis-Läsion behandelt wurden und an unserem Zentrum Beratung suchten.

Nervenfasertransfers oder kurz Nerventransfers innerhalb des Plexus werden vielfach als „Neurotisation“ bezeichnet. Das ist falsch. Unter Neurotisation versteht man das Einwachsen von Nervenfasern in denerviertes Gewebe. Bei einer Nervennaht wird der denervierte distale Stumpf vom normal innervierten proximalen Stumpf her neurotosiert. Ein denervierter Muskel kann auf verschiedene Weise neurotisiert werden. Beim Nervenfasertransfer werden Nervenfasern von einem funktionierenden Nerv auf einen denervierten Nerv transferiert, um diesen zu neurotisieren. Die Neurotisation ist das Ziel und der Transfer die Maßnahme, um das Ziel zu erreichen.
