Zusammenfassung

Bei extrakraniellen, meist chronischen Karotisläsionen sind dagegen chirurgische Eingriffe sowohl in der Indikationsstellung als auch der Operationstechnik seit langem standardisiert. Sie gehören zu den in Deutschland am häufigsten durchgeführten Arterienoperationen, wobei sich methodisch die Eversionsendarteriektomie durchgesetzt hat. Die Ergebnisse großer internationaler Studien sind exzellent und in den meisten Zentren den endovaskulären Verfahren oder der ausschließlich medikamentösen Therapie überlegen. Ausnahmen bilden Karotisdissektionen und periphere Aneurysmen der A. carotis interna. Bei den Vertebralisarterien sind die invasiven Therapieverfahren nicht so erfolgreich, weshalb hier die Indikation strenger gestellt werden muss.
