Zusammenfassung
Die Erkrankungen der extrakraniellen hirnversorgenden Arterien verdienen besondere Aufmerksamkeit, da sich in Deutschland ca. 170.000 Schlaganfälle pro Jahr ereignen und ca. 15–20 % dieser Ereignisse auf Stenosen oder Verschlüsse der extrakraniellen hirnversorgenden Gefäße zurückgeführt werden können. Der Prävention und Diagnostik carotisbedingter zerebraler Ischämien kommt somit eine große Bedeutung zu. Unverändert stellt die farbkodierte Duplexsonographie neben der Dopplersonographie die wichtigste apparative Untersuchungsmethode dar. Trotz zahlreicher Studien zur Therapie symptomatischer und asymptomatischer Carotisstenosen herrscht Unsicherheit und Uneinigkeit in der Wahl des Therapieverfahrens. Die Indikation zur invasiven Behandlung einer Carotisstenose sollte immer interdisziplinär unter Einbeziehung eines in der Diagnostik und Behandlung von Carotisstenosen erfahrenen Neurologen gestellt werden.
