Zusammenfassung
Cephalosporine sind bizyklische β-Laktam-Antibiotika mit 7-Amino-Cephalosporansäure als Grundgerüst. Wie Penicilline wirken sie bakterizid. Cephalosporine weisen charakteristische, gemeinsame Lücken im Wirkungsspektrum auf: Primär resistent sind Enterokokken, Listerien, Campylobacter, Legionellen, C. difficile, Mykobakterien, Mykoplasmen und Chlamydien. Einzelne Substanzen wirken auch gegen Anaerobier. Die Einteilung erfolgt nach Gruppen: Höhere Gruppen haben eine bessere Aktivität gegen gramnegative Stäbchenbakterien, gegen grampositive Bakterien ist diese z. T. geringer (Gruppe 3a, Ceftriaxon). Cephalosporine der Gruppe 3b wirken auch bei P. aeruginosa, solche der Gruppe 4 (Cefepim) haben zusätzlich eine starke Wirksamkeit im grampositiven Bereich. Sog. MRSA-Cephalosporine wie Ceftobiprol und Ceftarolin wirken auch gegen penicillinresistente Pneumokokken. Der massive klinische Einsatz der Cephalosporine hat wesentlich zur Selektion multiresistenter, ESBL-bildender („extended-spectrum β-lactamase“) Enterobakterien und Enterococcus faecium-Stämme beigetragen.
