Zusammenfassung
Gesunde Frauen, befragt nach ihren Erwartungen an ihre Schwangerschaft und Geburt antworteten: „Aufrechterhaltung der körperlichen und soziokulturellen Normalität, eine unauffällig verlaufende, gesunde Schwangerschaft, ein problemloser Übergang in effektive Geburtswehen und Geburt sowie eine positive Mutterschaft.“ Unbestritten ist, dass eine Schwangerenvorsorge im Vergleich zu keiner Vorsorge Leben retten kann. In Deutschland ist die Schwangerenvorsorge gesetzlich durch die sogenannten „Mutterschafts-Richtlinien“ des Gemeinsamen Bundesausschusses (G-BA) geregelt. Dabei ist „... das vorrangige Ziel der ärztlichen Schwangerenvorsorge ... die frühzeitige Erkennung von Risikoschwangerschaften und Risikogeburten.“ Besondere Erwähnung findet neben der Vermeidung unnötiger Kosten auch die Forderung, dass nur Maßnahmen angewendet werden sollen, deren diagnostischer und vorbeugender Wert ausreichend gesichert ist. Aber gerade unter diesen gibt es auch Maßnahmen, die nicht ausreichend untersucht oder gar evidenzbasiert sind. Das vorliegende Kapitel will etablierte Vorsorgeuntersuchungen unter dem Gesichtspunkt der Aktualität und der Evidenz kritisch beleuchten.
