Zusammenfassung
Die Thoraxdrainage (THD) stellt eine häufige, meist dringliche und oft auch ausreichende Maßnahme dar, um die initiale pulmonale Symptomatik zu entschärfen. Sie hat die Aufgabe Luft oder Flüssigkeiten aus der Pleurahöhle zu entleeren, die physiologischen Druckverhältnisse wiederherzustellen und die Lunge zur Entfaltung zu bringen. Dabei sind Pneumothorax, Pleuraerguss, Pleuraempyem, Hämothorax und die postoperative THD die häufigsten Indikationen. Während Nadelaspiration und kleinlumige THD, beim Pneumothorax und Pleuraerguss, unter sonografischer Evaluation erfolgen, ist der 4./5. Interkostalraum in der (vorderen) bis mittleren Axillarlinie die bevorzugte Inzisionsstelle der offen-chirurgischen THD. Sicher in der Durchführung ist die hier vorgestellte Schritt-für-Schritt-Methode. Blutungen mit Hämothorax, Fehllagen, Entzündungen, Lungenläsionen, Entfaltungsödem und Verletzungen intrathorakaler oder intraabdomineller Organe, stellen mögliche Komplikationen dar.
