Zusammenfassung

Thoraxverletzungen spielen in der Traumatologie ein grosse und wichtige Rolle, da sie potenziell bereits am Unfallort lebensbedrohlich sein können und die Prognose wesentlich von einer zeitnahen Diagnose und Behandlung abhängig ist. Wichtig dabei ist, dass nebst der Verletzung des Thorax inklusive Weichteile alle Organe innerhalb aber auch in der Nachbarschaft (z. B. Leber, Milz, Darm) betroffen sein können. Unmittelbare Folgen können schwere Gasaustausstörungen, grosse Blutungen und Kompartmentsyndrome wie (Spannungs-) Pneumothorax, Hämatothorax, Perikardtamponade sein, die sehr zeitnah behandelt werden müssen um u. U. fatale Konsequenzen abzuwenden. Diagnostisch spielt nebst der Klinik die Bildgebung (Röntgen, Computertomografie, Ultraschall bzw. Echokardiografie) eine grosse Rolle und muss deshalb sehr niederschwellig verfügbar sein. Therapeutisch stehen die Sicherstellung eines suffizienten Gasaustausches, die Drainage von Luft und Blut sowie eine gute analgetische Behandlung im Vordergrund.
