Präoperative kardiale Diagnostik in der Thoraxchirurgie

Zusammenfassung

Kardiale Komplikationen, wie Myokardinfarkt bzw. Myokardverletzung ohne Infarkt („myocardial injury“), Lungenödem, Kammerflimmern, akuter Herzstillstand oder plötzlicher Herztod, sind für ca. 40–50 % der perioperativen Morbidität und Mortalität bei Patienten mit nichtkardialen Operationen verantwortlich. Thorax- bzw. gefäßchirurgische Operationen zählen in diesem Zusammenhang zu den Hochrisikoeingriffen. Ein strukturiertes und individualisiertes präoperatives Risikomanagement berücksichtigt die kardiale Vorgeschichte, Medikation, strukturelle Herzerkrankungen, Komorbiditäten, Alter des Patienten, Biomarker und die Schwere des geplanten operativen Eingriffs. Etablierte Risiko-Scores und Stufen-Algorithmen helfen das prä- und perioperative Operationsrisiko einzuschätzen. Präoperative nichtinvasive Untersuchungen bleiben, auch unter Berücksichtigung von Umsetzbarkeit und Kosteneffektivität, im Wesentlichen Patienten mit einem bedeutsamen Risiko vorbehalten.
