Zusammenfassung
Nervenläsionen haben vielfältige Ursachen. Im Vordergrund stehen mechanisch bedingte Schäden des Nervs durch unmittelbare Verletzung oder Kompression. Letztere sind Themen dieses Buchs. Die Kenntnis anderer Ursachen einer Mononeuropathie ist aber unerlässlich, nicht zuletzt auch, um überflüssige chirurgische Eingriffe zu vermeiden. Nachfolgend werden mögliche Ursachen einer fokalen Neuropathie beschrieben.
Unter den physikalischen Ursachen von Nervenläsionen sind die seltenen Schäden durch Strahlen, Hitze, Frost oder Elektrotrauma (Blitzschlag) zu nennen. Auch toxische Effekte einer intraneuralen Injektion können Ursache einer umschriebenen Nervenschädigung sein. Zur Differenzialdiagnose gehören aber auch fokale Manifestationen systemischer Erkrankungen am peripheren Nerv wie die vaskulitische Mononeuritis multiplex, erregerbedingte Mononeuropathien, Mononeuropathien bei Diabetes mellitus oder Sarkoidose, die idiopathische Plexusneuritis (neuralgische Schulteramyotrophie) und die immunologischen Mononeuropathien. Eine erhöhte Vulnerabilität der Nerven gegenüber Druck kann bei Polyneuropathien, auch hereditären Ursprungs, auftreten. Schließlich gehören auch Nervenläsionen bei malignen Tumorleiden durch Infiltration oder Kompression zur Differenzialdiagnose.
