Zusammenfassung
Als Folge einer Ausscheidungsschwäche oder eines vermehrten Anfalls von Harnsäure kommt es bei der Gicht zu einer Erhöhung der Serumharnsäurekonzentration. Bei Überschreitung des Löslichkeitsprodukts für Harnsäure im Serum lagert sich die Harnsäure als Mononatriumuratkristalle im Gewebe ab, hier insbesondere in den Gelenken und den gelenknahen Weichteilstrukturen. Der Gichtanfall ist charakterisiert durch den akuten Beginn einer hoch schmerzhaften Arthritis eines Gelenks (meist Großzehengrundgelenk). Die chronische Gicht ist eine Arthritis mehrerer Gelenke, die unbehandelt nicht mehr verschwindet und längerfristig zu einer Zerstörung des Gelenks bzw. gelenknahen Knochens führt. Als Goldstandard der Diagnose einer Arthritis urica gilt der Nachweis von Mononatriumuratkristallen im Gelenk. Im Stadium der Hyperurikämie (ohne Gichtanfall) sollte in erster Linie die begleitende Erkrankung diagnostiziert und behandelt werden. Der Gichtanfall wird medikamentös behandelt. Spätestens nach dem zweiten Gichtanfall und bei der tophösen, chronischen Gicht ist die dauerhafte, effektive Harnsäuresenkung das Entscheidende jeder Therapie.
