Zusammenfassung
Die Arthritis urica, auch als Gicht bezeichnet, und die Chondrokalzinose, auch Pseudogicht genannt, verursachen eine Synovialitis durch ausfallende Urat- bzw. Kalziumpyrophosphat-Kristalle. Während die Chondrokalzinose, auch bei massiver Kristalleinlagerung in allen Geweben des Gelenkes, klinisch asymptomatisch bleiben kann, führt das Ausfallen und das Auflösen der Uratkristalle häufig zu sehr schmerzhaften Arthritiden. Vor der Menopause sind Frauen, welche keine Risikofaktoren aufweisen, vor der Gicht geschützt. Der erste Gichtanfall tritt typischerweise bei übergewichtigen Männern in der 3. bis 5. Lebensdekade nach dem Genuss purinreicher Kost in Verbindung mit Alkoholkonsum auf und betrifft häufig das Knie. Während die Gicht durch eine Diät und medikamentös behandelt werden kann, beschränkt sich die Therapie der Chondrokalzinose, so sie Schmerzen verursacht, auf die Symptome. Kristallarthropathien verlaufen in der Regel progredient und führen zur Gelenkdestruktion. Operationen kommen nur in Ausnahmefällen zur Anwendung.
