Zusammenfassung
MCN stellen prämaligne Läsionen dar, deren Entartungsrisiko bei ca. 15 % liegt. Daher sollten sie nach den aktuellen Konsensus-Leitlinien grundsätzlich operativ entfernt werden. Liegt entsprechend den präoperativen Befunden keine Risikokonstellation für bereits vorhandene Malignität vor, kann ein lokal begrenztes Verfahren (Enukleation, Pankreassegmentresektion, milzerhaltende Linksresektion) erfolgen. Bei Verdacht auf Malignität oder intraoperative Bestätigung im Schnellschnitt muss eine onkologische Resektion mit Lymphadenektomie durchgeführt werden. Eine strukturierte Nachsorge ist bei gutartigen MCN nicht erforderlich, da sehr selten Rezidive auftreten.
