Zusammenfassung
Wirbelsäulenverletzungen im Wachstumsalter sind selten, gehen jedoch häufiger als beim Erwachsenen mit neurologischen Störungen einher. Diese können auch ohne konventionell radiologisch fassbare Verletzungen vorkommen. Die Problematik der Röntgendiagnostik liegt v. a. in der Abgrenzung von Synchondrosen und Apophysen gegenüber Frakturlinien. Bei neurologischen Ausfällen ohne radiologisch fassbare Läsionen ist die Abklärung mittels Magnetresonanztomographie indiziert. Die Verletzungstypen entsprechen im Prinzip denjenigen der Erwachsenen, zusätzlich sehen wir Verletzungen der Endplatten (Wachstumsfuge) und der Ringapophysen. Stabile Kompressionsfrakturen werden konservativ behandelt. Die Fähigkeit zur spontanen Korrektur einer posttraumatischen Fehlstellung nimmt mit dem Alter ab: Bei den unter 10-jährigen Kindern ist die Wiederaufrichtungspotenz der posttraumatischen Kyphose ausgezeichnet, während Fehlstellungen in der Frontalebene nur zögerlich oder gar nicht remodellieren. Instabile Verletzungen sowie Verletzungen mit Kompression von neuralen Strukturen werden im Prinzip operativ behandelt.
Abstract
Spine injuries during growth are rare, but in comparison to adults they are more often associated with neurologic impairment. They also may occur without visible injuries in X-rays. The problems of conventional radiologic diagnostics include before all the differential diagnosis between synchondrosis, apophysis and fracture lines. MRI is indicated in case of neurologic deficits without radiologic abnormalities. In principle the fracture types correspond to those seen in adults. In addition growth specific injuries of the endplates (growth plates) or ring apophysis may occur. Stable compression fractures are treated conservatively. The spontaneous remodelling capacity for posttraumatic deformities decreases with age: in children below the age of ten years the remodelling capacity for posttraumatic kyphosis is excellent whereas deformities in the frontal plane show no or only incomplete remodelling. Unstable fractures and injuries with associated compression of neural structures should be treated conservatively.
