Doel
Bepalen welk deel van de mensen meedoet en het aantal nieuw opgespoorde aandoeningen tijdens de pilot implementatie van het PreventieConsult Cardiometabool risico (CMR).
Methode
Zestien huisartsenpraktijken hebben deelgenomen aan de pilot implementatie PreventieConsult CMR. Patiënten tussen de 45 en 70 jaar moesten met een onlinerisicoschatting het risico op cardiometabole aandoeningen bepalen. In de helft van de praktijken kregen patiënten een uitnodiging in de vorm van een persoonlijke brief en in de andere helft gebeurde dat via folders en een poster in de wachtkamer. Deelnemers met een verhoogde score op de risicoschatting kregen het advies om een afspraak te maken voor een praktijkconsult. We gebruikten de gegevens van de risicoschatting, de praktijkconsulten en de resultaten van een vragenlijst onder een steekproef van de doelpopulatie.
Resultaten
De respons op de uitnodigingsbrief en de poster en folders was respectievelijk 33% en 1%. In totaal hebben 617 personen de risicoschatting uitgevoerd, van wie er 392 (64%) een verhoogde score hadden. Dit resulteerde uiteindelijk in 142 patiënten op de praktijkconsulten. Bij 28 patiënten (20%) spoorden de huisartsen een of meer cardiometabole aandoeningen op.
Conclusie
Eenderde van de genodigden heeft geparticipeerd in de risicoschatting van het PreventieConsult CMR, van wie meer dan de helft een verhoogde risicoscore had. Wanneer mensen met een verhoogde risicoscore op de risicoschatting bij de huisarts komen, blijkt een op de vijf een cardiometabole aandoening te hebben.
Abstract

Purpose
To investigate the participation of general practice patients in a voluntary cardiometabolic health check-up (PreventieConsult CMR) and the number of newly diagnosed cardiometabolic disorders.
Methods
The PreventieConsult CMR was used and evaluated in 16 general practices. Patients between 45 and 70 years old without cardiometabolic disorders were invited to fill in an online questionnaire, the answers of which were used to calculate their risk (low, medium, or high) of cardiometabolic disorders. Half of the participating practices invited patients by means of a personal letter and the other half by means of leaflets and posters in the practice waiting room. Patients at high risk of cardiometabolic disorders were advised to consult their general practitioner. Data from the online risk questionnaire, GP consultation, and a questionnaire sent to a random sample of participating patients for a non-response analysis were analysed.
Results
The response to the invitation letter and to the posters and leaflets was 33% and 1%, respectively. In total, 617 patients completed the online risk questionnaire, 392 (64%) of whom were at high risk of cardiometabolic disorders. In total, 142 patients consulted their GP and 28 (20%) were found to have one or more cardiometabolic disorders.
Conclusion
About one-third of patients who were invited to participate took part in the PreventieConsult CMR pilot study, and half of these patients were considered at high risk and required further evaluation by their GP. Cardiometabolic disorders were diagnosed in 1 of 5 high-risk patients who consulted their GP.
