Fowler NO: Cardiac tamponade. A clinical or an echocardiographic diagnosis? Circulation 1993, 87:1738–1741. In most patients, cardiac tamponade should be diagnosed by means of a clinical examination, which shows elevated systemic venous pressure, tachycardia, dyspnea, and paradoxical arterial pulse. The diagnosis is confirmed by echocardiographic demonstration of moderately large or large circumferential pericardial effusion and, in most instances, right atrial compression as well as abnormal respiratory variation in right and left ventricular dimensions and in tricuspid and mitral valve flow velocities.

The technique and pitfalls of recording, the criteria for the diagnosis of pericardial effusion, and the sensitivity of these criteria are discussed. Echocardiographic findings in special instances (eg, accumulation of pericardial effusion behind the left atrium, the swinging heart syndrome, and cardiac tamponade) are also discussed.

Palacios IF, Tuzcu EM, Ziskind AA, et al.: Percutaneous balloon pericardial window for patients with malignant pericardial effusion and tamponade. Cathet Cardiovasc Diagn 1991, 22:244–249. Percutaneous balloon pericardial window creation was performed in eight patients with malignant pericardial effusion and tamponade. It was shown to be a useful and safe technique that can be used to avoid surgery in these patients.

J Am Coll Cardiol 1993, 21:1–5. Percutaneous balloon pericardiotomy was successful in helping to manage large pericardial effusions, particularly in patients with a malignant condition. It may become the preferred treatment to avoid a more invasive procedure for patients with pericardial effusion and a limited life expectancy.
