Zusammenfassung
Organisationssoziologische Studien weisen darauf hin, dass Handeln in Organisationen weder durch Webers Modell der rationalen Herrschaft noch durch das ökonomische Modell des Rational Choice hinreichend beschrieben werden kann. Vielmehr sind die unterschiedlichen Sphären der Organisation nur lose miteinander gekoppelt. Insbesondere im Krankenhaus, das dem gesellschaftlichen Zentralwert Gesundheit verpflichtet ist, bilden die ökonomische Forderung nach Kosteneffizienz und die Eigengesetzlichkeiten der medizinischen Handlungslogik unterschiedliche Sphären, die innerhalb der Organisation miteinander in Beziehung gesetzt werden müssen. Auf der Basis einer qualitativ rekonstruktiven Längsschnittstudie wurden die Wandlungsprozesse in zwei städtischen Krankenhäusern durch Feldstudien in den Jahren 2000/2001 und 2004 untersucht. Wenngleich auf einer Ebene allein schon die massiven Personalkürzungen eine Reorganisation der ärztlichen Arbeit notwendig werden ließen, zeigen sich auf der anderen Seite innerhalb der ärztlichen Orientierungen weiterhin Freiheitsgrade, die es erlauben, einer originär medizinischen Logik die Bahn zu brechen. Es deutet sich jedoch an, dass der Konflikt zwischen den medizinischen und den ökonomischen Aspekten der Krankenbehandlung nicht mehr nur intraorganisatorisch, sondern auch in dem die Organisation übergreifenden Behandlungsnetzwerk ausgetragen wird.
Abstract
The following paper is based on ethnographic studies in German hospitals and focuses on medical actions and decision-making. As the investigation was conducted as a panel, it is possible to perceive of how the organizational changes have affected the medical work in hospitals. In the first part of the study, from 2000 to 2002, field research took place in four different types of departments in several hospitals. In the second part of the study, in the year 2004, the general ward and the department of abdominal surgery were revisited. In the time period between the two field-research phases the hospitals in Germany were experiencing an extensive process of organizational change. On the one hand, the introduction of the new “Diagnose Related Groups” (DRG) brought a new cost accounting structure. The base for calculating the cost is now no longer length of hospital stays, but a billing system related to diagnosis. On the other hand, connected to the ongoing privatization of hospitals, concepts of modern management (computer based “controlling”, “outsourcing” and centralization of important functions) are beginning to enter into hospital life. These processes are significantly changing the context of medical work and also the way in which medical decisions are processed.

Les études de sociologie des organisations montrent que ni le modèle de la domination rationnelle de Weber, ni le modèle économique du choix rationnel ne rendent suffisamment compte de l’action dans les organisations. Les différentes sphères de l’organisation ne sont au contraire reliées entre elles que de façon lâche. En particulier à l’hôpital, l’exigence d’efficience économique et les lois propres à la logique de l’action médicale forment différentes sphères qui doivent être mises en relation au sein de cette organisation au service cette valeur centrale qu’est la santé. Les processus de transformation dans deux hôpitaux municipaux ont été étudiés sur la base d’une étude longitudinale qualitative-reconstructive à l’aide d’enquêtes de terrain réalisées en 2000/2001 et en 2004. Bien que les réductions massives de personnel aient à elles seules rendu nécessaire une réorganisation du travail médical, les orientations médicales conservent des degrés de liberté permettant de frayer la voie à une logique proprement médicale. Il apparaît toutefois que le conflit entre les aspects médicaux et économiques du traitement des malades n’a plus seulement lieu au niveau intraorganisationnel mais aussi au niveau du réseau de traitement dans lequel s’inscrit l’organisation.
