Summary
During recent years the therapeutic indications of monoclonal antibodies are on the increase. Most monoclonal antibodies are immunosuppressants. Thus, therapeutic successes are accompanied by an increase of serious infections. Closest cooperation between the haematologist and oncologist and the infectious-diseases specialist is a prerequisite for the successful outcome of treatment of the individual patient. Rituximab and alemtuzumab especially pave the way for a variety of opportunistic infections. Prophylactic drugs directed against bacterial infections and invasive fungal infections have not been proven to be useful. Although the efficacy of prophylactic cotrimoxazole against Pneumocystis carinii pneumonia has not been thoroughly examined in this setting, it is usually given at a dose of two double-strength tablets every other day. Whether reactivation of herpes simplex and varizella zoster can be influenced prophylactically has not been studied in randomised trials. In case of fever and detection of cytomegalovirus reactivation, preemptive therapy should be given.
Zusammenfassung
Seit einigen Jahren nehmen Anzahl und Indikationsgebiete therapeutisch eingesetzter monoklonaler Antikörper rasch zu. Die überwiegende Zahl entfaltet eine immunsuppressive Wirkung, sodass klinische Erfolge in zunehmendem Maße schwerwiegenden Infektionen gegenüber stehen. An diesem Schnittpunkt der Hämatologie und Onkologie und der Infektiologie ist die enge Zusammenarbeit zwischen beiden Disziplinen Voraussetzung für den erfolgreichen Verlauf einer antineoplastischen Therapie. Insbesondere Rituximab und Alemtuzumab bereiten einem breiten Spektrum opportunistischer Infektionen den Weg. Medikamentöse Prophylaxen gegen bakterielle Infektionen und gegen invasive Mykosen haben sich bislang nicht als effektiv erwiesen. Die Wirksamkeit einer Prophylaxe der Pneumocystis-carinii-Pneumonie ist nicht eindeutig belegt. Letztere wird jedoch zumeist mit Cotrimoxazol 2-mal 960 mg jeden zweiten Tag durchgeführt. Ob Reaktivierungen von Herpes simplex und Varizella zoster prophylaktisch beeinflussbar sind, ist nicht randomisiert geprüft worden. Bei Fieber unbekannter Ursache und gleichzeitigem Nachweis einer CMV-Reaktivierung sollte eine präemptive Therapie erfolgen.
