15-jährige Erfahrung mit der subtalaren Schrauben-Arthrorise beim kindlichen Plattfuß

Abstract

Our experience with the subtalar screw arthroereisis for correction of flat foot in children is reported. An AO cancellous bone screw is implanted in the sinus tarsi, laterally in the os calcis, blocking the abnormal valgus of the foot. Between 1990 and 2004, this technique was used on 152 children, 74 of them bilateral, for a total of 226 feet. There were 82 boys and 70 girls, with an average age of 10.6±1.9 years. The results were good in 95.4% of the cases, while there were complications in 4.6% of the cases. In the well-corrected feet, up to now, 55 implanted screws have been removed. The removal of the screw has occurred on average 2.9 years after its implant. The indications and contraindications are as follows. Indications: pes calcaneovalgus, which develops into a flat foot (the spontaneous correction can be expected until year 10–11); juvenile flat foot with medial protrusion of the talar head and complete absence of the longitudinal arch; symptomatic pes calcaneovalgus, with pain on the insertion of the tibialis posterior tendon; minimum age of 6 years, if a correction with conservative treatment does not show any improvement; maximum age of 12–13 years, considering that there always has to be a manual reducibility of the deformity. Contraindications: posttraumatic flat foot; congenital flat foot; stiff flat foot; age over 13 years.

Zusammenfassung

Für die operative Behandlung des kindlichen Knicksenkfußes und Plattfußes, stehen einige Operationstechniken zur Verfügung. Bedingt durch ihre Minimal-invasivität hat die subtalare Schrauben-Arthrorise an Bedeutung zugenommen. Eine AO-Spongiosaschraube wird in Höhe des Sinus Tarsi, lateral im Kalkaneus, zur Blockierung des abnormalen Rückfußvalgus, implantiert. Das Operationsverfahren wurde in den Jahren 1990–2004 bei 152 Kindern, bei 74 beidseitig, insgesamt also 226-mal verwendet. Es waren 82 Jungen und 70 Mädchen mit einem durchschnittlichen Alter von 10,6±1,9 Jahren. Gute Ergebnisse sind in 95,4% der Fälle zu vermerken, während Komplikationen in 4,6% der Fälle auftraten. Von den eingebrachten Schrauben wurden bei weiterbestehender Korrektur, bis heute, 55 entfernt. Die Schraubenentfernung erfolgte durchschnittlich 2,9 Jahre nach ihrer Implantation. Die Indikationen und die Kontraindikationen werden wie folgt zusammengefasst. Indikationen: kindlicher Knick-Senkfuß mit der Entwicklung zum Plattfuß (die spontane Korrektur kann bis zum 10–11 Lebensjahr abgewartet werden), kindlicher Plattfuß mit medialer „Protrusion“ des Taluskopfes und kompletter Aufhebung des Längsgewölbes, kindlicher Knicksenkfuß mit Schmerzen im Bereich des Ansatzes der Sehne des M. Tibialis posterior; untere Altersgrenze: 6 Jahre, wenn durch konservative Maßnahmen eine Korrektur nicht mehr zu erwarten ist, obere Altersgrenze: 12–13 Jahre. Voraussetzung ist in jedem Fall eine manuelle Korrigierbarkeit der Deformität. Kontraindikationen: posttraumatischer Plattfuß, angeborener Plattfuß, kontrakter Plattfuß, Alter über 13–14 Jahre.
