Zusammenfassung
Die Tracheotomie stellt einen der ältesten chirurgischen Eingriffe in der Geschichte der Medizin dar und wird als elektiver Eingriff im Atemwegsmanagement – insbesondere bei langzeitbeatmeten Intensivpatienten – zunehmend häufiger durchgeführt. Der Notarzt kann daher immer häufiger mit tracheotomierten Patienten im Rahmen des Interhospitaltransfers, aber auch mit Notfällen bei Tracheostomaträgern im häuslichen Bereich konfrontiert werden. Nachfolgend werden die verschiedenen Techniken der Tracheotomie, der Stellenwert von Tracheotomie und Koniotomie im notfallmäßigen Atemwegsmanagement sowie ärztliche Handlungsstrategien bei typischen Komplikationen und Notfällen diskutiert.
Abstract
Tracheostomy is one of the oldest surgical procedures in the history of medicine and, in recent years, is increasingly being performed to gain long-term airway access. In particular, percutaneous approaches have become the standard treatment modality in the airway management of critically ill patients on long-term ventilation. The emergency physician can be confronted with patients with a tracheostomy during interhospital transfer, as well as in case of out-of-hospital emergencies. The present article reviews and discusses the different techniques of tracheostomy, the role of tracheostomy and cricothyrotomy for emergency airway control, and the management of emergencies in patients with tracheostomy.
