Zusammenfassung
Bildgebende Verfahren wie die Sonographie und Magnetresonanztomographie haben das Spektrum der rheumatologischen Diagnostik durch „Sichtbarmachung“ pathomorphologischer Veränderungen erheblich erweitert. Die Vorteile dieser apparativen Techniken ergeben sich aus der Schichtanalyse und drei-dimensionalen Darstellung, welche eine differenziertere Beurteilung von entzündlichen Weichteilstrukturen, sowie eine sensitivere und somit frühere Detektion knöcherner Gelenkveränderungen erlauben. Neueste diagnostische Techniken und Methoden wie die Miniarthroskopie, der Farb- bzw. Power-Doppler-Ultraschall und die Positronen-Emissions-Tomographie (PET) ergänzen sinnvoll das diagnostische Instrumentarium des Rheumatologen, da wo Routineverfahren an ihre Grenzen stoßen. Die Doppler-verstärkte Gefäßsonographie als auch die PET erfassen und messen lokal als auch komplex entzündliche Veränderungen im Rahmen von Systemerkrankungen, welche durch bisherige diagnostische Standardverfahren nur schwer erkennbar waren. Die Miniarthroskopie, als minimal-invasives Verfahren, erlaubt ferner neben der gezielten Diagnostik an einem Gelenk auch den direkten Zugang zum pathologischen Substrat, mit der Möglichkeit der Biopsie und weiterführender molekularbiologischer Analytik. Der Rheumatologie eröffnen sich mit der Integration dieser diagnostischen Techniken neue Perspektiven zur Ätiopathogenese, Frühdiagnostik und Verlaufskontrolle entzündlich-rheumatischer Krankheitsbilder.
Summary
Imaging procedures, such as ultrasound and magnetic resonance imaging have broadened the diagnostic spectrum by ”visualization” of pathomorphologic changes in rheumatic diseases. The advantages of these techniques are sectional imaging and three dimensional illustration, which enhance the exact detection of inflammatory soft tissue alterations and the sensitive and early detection of bony changes. Most current techniques and methods such as miniarthroscopy, colorand power doppler ultrasonography and positron emission tomography (PET) complete the set of diagnostic tools for rheumatologists, when other diagnostics reach their limits. Doppler ultrasonography and PET detect and assess inflammatory changes of systemic diseases locally and on the whole, which were not visible by standard diagnostic procedures. Miniarthroscopy furnishes, besides the direct evaluation of the joint, access to the pathomorphologic substrat itself with the opportunity of tissue sampling and further molecular biological analysis. These new diagnostic techniques open a widespread goal for rheumatologists in terms of etiopathogenesis, early diagnosis and monitoring of inflammatory rheumatic diseases.
