Zusammenfassung
Das akrale myxoinflammatorische fibroblastische Sarkom ist ein niedrigmalignes Sarkom der distalen Extremitäten und wurde erstmals von Meis-Kindblom und Kindblom 1998 beschrieben. Die hier vorgestellten 6 Fälle traten bei 4 männlichen und 2 weiblichen Patienten im Alter von 22–60 Jahren (im Mittel 42,8 Jahre) auf. Als Lokalisationen der 1–4 cm (im Mittel 2,4 cm) in der größten Ausdehnung messenden Läsionen wurden der Fußrücken und die Finger (jeweils 2 Fälle) sowie in jeweils 1 Fall das Handgelenk und der Oberarm angegeben. Die Patienten suchten nach kurzer Zeit (im Mittel 3 Monate) wegen einer schmerzlosen Schwellung den Arzt auf. Die Tumoren waren unscharf begrenzt und wuchsen infiltrierend im subkutanen Fettgewebe, in je 1 Fall wurden sekundär auch die Dermis und die Muskulatur einbezogen. Histologische Merkmale waren das multinoduläre Wachstum, ein mäßig pleomorphes Zellbild mit myxoiden und fibrosierten, teilweise hyalinisierten Arealen, die Existenz von großen Zellen mit prominenten Nukleolen sowie eine wechselnd starke entzündliche Infiltration, die vorwiegend aus Lymphozyten und Histiozyten bestand. Etwa 2 Monate nach Exzision traten in 2 Fällen Lokalrezidive des Tumors auf, Metastasen wurden nicht beobachtet. Eine differenzialdiagnostische Abgrenzung ist besonders vom inflammatorischen Pseudotumor und von höhermalignen Sarkomen erforderlich.
Abstract
Acral myxoinflammatory fibroblastic sarcomas are low-grade sarcomas of the distal extremities, first described by Meis-Kindblom and Kindblom in 1998. In our series of six cases this sarcoma occurred in four men and two women aged 22–60 years (mean 42.8). The tumors measured 1–4 cm (mean 2.4) and were localized in the foot (two cases), finger (two), wrist (one), and upper arm (one). The patients had a short history of a painless mass (mean duration 3 months). The tumors were poorly circumscribed, and infiltrated the subcutaneous fat and in one case each also the dermis and musculature. Histological features of the tumors were a multinodular configuration with infiltrative margins, myxoid and fibrotic zones with hyalinized areas, moderately pleomorphic cells with large cells showing a prominent nucleolus and an inflammatory infiltrate consisting of lymphocytes and histiocytes. Two months after excision the tumor recurred locally in two cases; no metastases were observed. Differential diagnosis is especially necessary from inflammatory pseudotumors and more aggressive sarcomas.
