Zusammenfassung
Fragestellung. In einer retrospektiven Studie werden Ergebnisse nach konservativer und operativer Behandlung thorakolumbaler Frakturen in Abhängigkeit von ihrer Höhenlokalisation dargestellt.
Methodik. In 2 Jahren wurden 70 Patienten mit A1-/A2-Bruch konservativ behandelt und 38 Patienten wegen A3-/B-/C-Verletzung mit Fixateur interne versorgt.Zur Auswertung wurde eine Einteilung in 3 Wirbelsäulenabschnitte vorgenommen (Th5–Th10,Th11–L2, L3–L5).Die Nachuntersuchung erfolgte mindestens 1 Jahr nach Implantatentfernung oder Abschluss der konservativen Behandlung.
Ergebnisse. Der Korrekturverlust war thorakal am größten, im unteren Lendenwirbelbereich am geringsten.Bei konservativer Therapie fand die Sinterung zu 3/4 im Wirbel statt, bei operativer Behandlung vorrangig in den benachbarten Zwischenwirbelräumen.Es gab keine generelle Korrelation mit der Beschwerdesymptomatik.
Schlussfolgerung. Aufgrund dieser Ergebnisse werden A1/A2-Frakturen der oberen BWS (bis Th10) mit Kyphose >15° ventral instrumentiert, sonst jedoch frühfunktionell behandelt. A3-Frakturen der thorakalen Region und des thorakolumbalen Überganges werden ventral, in der unteren LWS (ab L3) dagegen dorsal instrumentiert. B- und C-Verletzungen instrumentieren wir kombiniert dorsoventral.
Abstract
Question. This retrospective study presents results after conservative and operative treatment of thoracolumbar fractures as function of its localization.
Methodology. In 2 years 70 patients with A1/A2 fracture were conservatively treated, 38 patients with A3/B/C injury were treated by internal fixtor.For evaluation 3 vertebral sections(Th5–10,Th11–L2,L3–5)were defined. Follow-up took place 1 year after implant removal or end of conservative treatment.
Results. The correction-loss was highest in thoracic,lowest in lumbar region.After conservative therapy,correction-loss was located to 3/4 in vertebra itself,after operative treatment especially in adjacent disc spaces.There wasnogeneral correlation tocomplaints.
Conclusion. In consequence of these results A1/A2-fractures in the upper thoracic spine (<Th10) with kyphosis >15° will be stabilized anteriorly, in other regions functional treated. A3-fractures of thoracic spine and thoracolumbar junction will be operated from anterior, in lower lumbar spine (>L3) from dorsal. B- and C-injuries should be instrumented with a combined dorsoventral procedure.
