Zusammenfassung
□ Begriffsbestimmung
Seit Ende der 60er Jahre wurden in den USA vorsorgliche Verfügungen („advance directives”) entwickelt, in denen ein Patient Verfügungen über seine medizinische Behandlung für den Fall seiner Einwilligungsunfähigkeit formulieren kann. In Deutschland wird zwischen 1. Patientenverfügung (auch Patiententestament oder Patientenbrief), 2. Betreuungsverfügung und 3. Vorsorgevollmacht differenziert.
□ Empirische Untersuchungen
In empirischen Studien, überwiegend aus den USA, wurden Patientenverfügungen im Hinblick auf ihre Verwendungshäufigkeit in der klinischen Praxis untersucht. Dabei finden die Einstellung der Ärzte zu Patientenverfügungen sowie die Wünsche von Patienten, Angehörigen und der Allgemeinbevölkerung besonderes Interesse. Die Übersichtsarbeit referiert die Ergebnisse im Text und in Tabellen.
□ Schlußfolgerung
Entgegen dem Wunsch von Bevölkerung und Patienten werden Patientenverfügungen in der klinischen Praxis relativ selten angewandt. Therapeutische Aufklärungsgespräche werden von Ärzten häufiger und intensiver durchgeführt als Gespräche über Therapiebegrenzung. Dabei wird eine Chance vergeben, über palliativmedizinische Maßnahmen zu informieren sowie Werte und Wünsche des Patienten an seinem Lebensende zu berücksichtigen.

□ Definitions
In the United States advance directives were developed since the late 60s in order to improve patients’ medical decision making in case of incompetence in the future. In Germany 3 types of advance directives exist: 1. “Patientenverfügungen”, 2. “Betreuungsverfügungen” and 3. “Vorsorgevollmachten”.
□ Empirical Studies
In empirical studies, mostly from the US, the implementation of advance directives into clinical practice and the attitude of physicians, patients, relatives and the general population have been examined. This review article provides results of theses studies in text and tables.
□ Conclusion
Against the will of patients and of the general population advance directives are not often used in clinical practice. Physicians talk with patients about treatment procedures more often than they talk about end of life decisions. Informing patients about advance directives opens the chance to improve palliative care and respects their choices and values at the end of their lives.
