Résumé
Cette étude a permis de comparer deux groupes de suicidants admis à l'hÔpital général en fonction de deux stratégies thérapeutiques partiellement différentes. Le «Groupe systématiquement traité (GST)», formé de 143 suicidants, comportait, outre les prises en charge classiques (thérapie de soutien, hospitalisation psychiatrique, intervention de crise auprès de couples ou de familles, intervention combinée, psychothérapie analytique, etc.), la mise sur pied de contrÔles ambulatoires (quelque jours après la sortie de l'hÔpital, après un mois, trois mois, six mois, un an, deux ans). Le «Groupe de référence (GR)», formé de 145 suicidants, ne comportait quant à lui que les prises en charge classiques. Tous les suicidants des deux groupes furent recontactés deux ans après leur tentamen. Les résultats obtenus suggèrent que la politique des contrÔles est probablement responsable de la différence du nombre de récidives de tentative de suicide et de suicides enregistrés entre le GST et le GR. Cette étude a également permis, d'une manière plus indirecte, de mieux analyser les problèmes relationnels entre suicidants et soignants et entre soignants eux-mÊmes, et leur impact sur la prise en charge des suicidants. Il en est résulté une série de modifications des conduites des soignants dans l'approche des suicidants et de leur entourage dès l'admission à l'hÔpital général, et la mise sur pied d'une recherche clinique à visée véritablement interdisciplinaire.
Zusammenfassung
Bei dieser Untersuchung werden zwei Gruppen von Suizidanten verglichen, die im allgemeinen Spital gemäss zwei teilweise verschiedener therapeutischer Strategien aufgenommen wurden: die eine, für die «systematisch behandelte Gruppe (SBG)», die 143 Suizidanten umfasste, besorgte neben den üblichen Betreuungsformen (Stütztherapie, psychiatrische Hospitalisierung, Krisenintervention bei Ehepaaren oder Familien, kombinierte Intervention, analytische Psychotherapie usw.) die Einrichtung ambulanter Kontrollen (ein paar Tage nach Spitalverlassen, nach einem Monat, nach drei Monaten, sechs Monaten, einem Jahr, zwei Jahren). Für die «Kontrollgruppe (KG)», die aus 145 Suizidanten bestand, wurden hingegen nur die üblichen Betreuungsformen eingesetzt. Mit allen Suizidanten der beiden Gruppen wurde zwei Jahre nach ihrem Selbstmordversuch erneut Kontakt aufgenommen. Die erhaltenen Ergebnisse lassen schliessen, diese Kontrollenpolitik sei wahrscheinlich für den Unterschied in der Anzahl der Rückfälle bei Selbstmordversuchen und Selbstmorden zwischen der SBG und der KG verantwortlich.
Die Untersuchung hat ebenfalls auf indirekte Weise erlaubt, die Beziehungsprobleme zwischen den Suizidanten und dem Pflegepersonal und unter den Pflegern selbst sowie ihren Einfluss auf die übernahme der Suizidanten besser zu analysieren. Daraus entstand nun eine Reihe von Abänderungen im Verhalten der Pfleger den Suizidanten und deren Umgebung gegenüber, schon bei der Aufnahme im allgemeinen Spital. Weiter ist auch eine klinische Untersuchung mit tatsächlich interdisziplinärer Absicht unternommen worden.
Summary
This is a comparative study of two groups of suicide attempters admitted in a general hospital, who were treated in different ways. The 143 attempters of the “systematically treated group (STG)” were proposed the classical therapeutic measures (supportive psychotherapy, psychiatric hospitalization, crisis interventions within couples or families, mixed interventions with the above treatments, psychoanalytically oriented psychotherapy, etc.), plus ambulatory controls (a few days after discharge from the general hospital, after one month, three months, six months, a year and two years). The 145 attempters of the “reference group (RG)” were proposed the classical measures only. All the attempters of both groups had a follow-up after two years. The results suggest the controls to be probably responsible for the difference of relapses and committed suicides between the two groups.
More indirectly, this study facilitated a further analysis of the relational problems between suicide attempters and staff, and among staff members themselves. The consequences were a modification of some attitudes of the staff toward the attempters and their significant others, and a new collaboration for an interdisciplinary clinical research.
