Summary
Functional results after proximal humeral replacement have been unsatisfactory and extensive resection at the shoulder for tumour leaves an unstable upper extremity. Therefore a joint implant is desirable which provides for particular mechanical and functional features of the shoulder joint. Criteria for the construction of the implant and for the implantation are mobility, stability, fixation and geometry. Using these criteria, the Stanmore-shoulder, the Leeds-shoulder and a third implant are evaluated.
Zusammenfassung
Die funktionellen Ergebnisse nach Teilarthroplastik durch Humeruskopfersatz einerseits und die Möglichkeiten der Extremitätenerhaltung bei ausgedehnten Resektionen im Bereich der Schulter andererseits lassen die Möglichkeit des totalen Schultergelenkersatzes wünschenswert erscheinen. An eine Schultergelenksendoprothese sind wegen der funktionellen und mechanischen Besonderheiten des zu ersetzenden Gelenks besondere Anforderungen zu stellen. Die funktionell-mechanischen Kriterien für die Konstruktion des Implantates sowie für die Implantation sind die der Gelenksgeometrie, der Mobilität, der Stabilität sowie der Qualität der Verankerung. Am Beispiel der Stanmore-Schulter, der Leeds-Schulter sowie einer eigenen Entwicklung werden die Möglichkeiten des Schultergelenkersatzes dargestellt. Die Besonderheiten der besprochenen Endoprothesen werden nach den erwähnten Kriterien kritisch gewertet und verglichen.
