Summary
We have used wire mesh in association with PMMA cement in six patients to reinforce a deficient medial acetabular wall. None of these cups show signs of loosening. We therefore investigated experimentally the mechanical characteristics of HDP cups embedded in cement reinforced with wire mesh. The mesh appears to increase the resistance of the cement to tensile stress and associated loading forces at the bone/cement interface by the “stiffening” affect of the metal framework.
Résumé
L'étude des prothèses cimentées ayant un recul de plus de 10 ans nous a fait découvrir, par l'observation, une règle pourtant parfaitement connue des ingénieurs: l'utilisation d'une armature, d'un ferraillage noyé au sein du ciment ou du béton améliore considérablement sa résistance aux contraintes en traction. Ce travail nous a permis de confirmer certaines hypothèses:

L'adjonction d'un grillage noyé dans le ciment améliore la résistance du polyméthylméthacrylate, en particulier en traction où il est par nature fragile.

Elle n'améliore pas l'ancrage à l'os, ce qui n'a rien d'étonnant.

On peut encore améliorer ces propriétés en utilisant un grillage plus épais, la seule limite étant sa malléabilité qui doit être suffisante pour épouser la forme du cotyle.

L'adjonction d'un deuxième grillage périphérique, augmente encore un peu la résistance mais surtout pourrait avoir un effet filet contre le relargage de PMMA.

La rigidification apportée par l'armature améliore très certainement la répartition des contraintes à l'interface os-ciment.
Ce travail a surtout jeté les bases d' expérimentations futures visant à confirmer l'amélioration de la résistance à la fatigue apportée par le grillage, à rechercher le diamètre et le maillage optimal pour permettre de développer un nouveau modèle de cotyle en polyéthylène lui même solidaire d'un grillage destiné à être noyé dans le ciment.
