Summary
The purpose of this study is to analyze the results of anorectal manometry and to evaluate the merits and disadvantages of this technique for the diagnosis of Hirschsprung’s disease. Studies were performed in 268 patients with constipation, including 95 cases of Hirschsprung’s disease. It is concluded from the results that Hirschsprung’s disease can be confidently diagnosed by manometric studies. If the studies are performed carefully with a suitable probe, reliability is over 95%. Manometry is the most useful method to differentiate Hirschsprung’s disease from other conditions, such as extremely short segment aganglionosis, colonic stenosis, and idiopathic megacolon.
Zusammenfassung
In dieser Arbeit sollen die Ergebnisse der anorektalen Manometrie und deren Vor- und Nachteile bei der Diagnose des M. Hirschsprung analysiert werden. Elektromanometrische Untersuchungen wurden bei 268 Kindern mit Obstipation durchgeführt, unter denen sich 95 mit Hirschsprung-Krankheit fanden. Aus den Ergebnissen wird der Schluß gezogen, daß der M. Hirschsprung durch die manometrischen Untersuchungen zuverlässig diagnostiert werden konnte. Wenn die Untersuchungen sorgfältig und mit einer geeigneten Sonde durchgeführt werden, liegt die Zuverlässigkeit dieser Methode bei über 95%. Die Manometrie ist die am besten geeignete Methode, den M. Hirschsprung von anderen pathologischen Veränderungen wie ein extrem kurzes aganglionäres Segment, eine Kolonstenose oder ein idiopathisches Megakolon abzugrenzen.
Résumé
Cette étude a pour but d’analyse l’évaluation de la manométrie anorectale et la comparaison de ses avantages et ses inconvénients dans le diagnostic de la maladie de Hirschsprung. Nous avons pratiqué des examens électro-manométriques sur 268 enfants atteints de constipation et avons décelé 95 cas de maladie de Hirschsprung. Les résultats indiquent que ces examens ont permis de diagnostiquer cette affection de façon fiable. A la condition d’une pratique très minutieuse de ces examens et de l’utilisation d’une sonde adéquate, la fiabilité de cette méthode est de plus de 95%. La manométrie est la méthode de choix à la frontière entre la maladie de Hirschsprung et les autres pathologies voisines telles qu’un segment aganglionnaire très court, une sténose du côlon ou encore un mégacôlon idiopathique.
