Summary
Anorectal manometry was performed in 48 Japanese children with Hirschsprung’s disease and 61 normal children. The resting pressure of the rectum and anal canal was not significantly different between these groups of subjects. The frequency of rhythmical contractions of the anal canal of patients was significantly lower than for the normal subjects, but the frequencies overlapped considerably. Therefore, the frequency is an inadequate indicator for identifying these patients.
Conventional manometry elicited a distinct rectoanal relaxation reflex from 90% of the normal children, and the rate increased to 98% when indistinct reflexes were regarded as positive. Indistinct reflexes often occur in neonates, possibly because the constriction of the anal canal is weak. However, when Prostaglandin F2a was intravenously administered during the examination, all ambiguous reflexes became distinct.
Of patients with Hirschsprung’s disease, 4% had a distinct reflex and 19% an atypical one. Most of the atypical reflexes were regarded as being artifacts and were mostly attributed to distension by a balloon. In these patients, the reflex was abolished in case of examination with electric Stimulation or Stimulation with cold water, procedures which do not dilate the rectum. Moreover these atypical reflexes did not fit the criteria for the normal rectoanal relaxation reflex prepared by the Japan Study Group of Pediatric Intestinal Manometry. The use of electric Stimulation, cold water, or intravenously administered Prostaglandin F2a improves reliability of the conventional anorectal manometry. A clear and accurate definition of the normal reflex should aid in excluding the atypical reflex.
Zusammenfassung
Bei 48 Kindern mit Hirschsprung-Krankheit und 61 gesunden Kindern in Japan wurden anorektale Manometrien durchgeführt. Der Ruhedruck im Rektum und Analkanal war nicht significant verschieden in beiden Gruppen. Die Häufigkeit der rhythmischen Kontraktionen des Analkanals war signifikant niedriger bei Kindern mit M. Hirschsprung als bei gesunden Kindern, jedoch zeigten beide Gruppen beträchtliche Überlappungen. Daher ist die Häufigkeit der rhythmischen Kontraktionen kein geeigneter Parameter zur Identifizierung der Kinder mit M. Hirschsprung.
Die konventionelle Manometrie ergab einen spezifischen rektoanalen Erschlaffungsreflex bei 90% der gesunden Kinder und diese Zahl wuchs auf 98% an, wenn unspezifische Reflexe mit einbezogen wurden. Unspezifische Reflexe werden häufig bei Neugeborenen beobachtet, möglicherweise weil die Konstriktion des Analkanals noch schwach ausgeprägt ist. Wenn jedoch während der Untersuchung Prostaglandin F2α intravenös verabreicht wurde, wurden aus den unspezifischen Reflexen spezifische.
4% der Kinder mit M. Hirschsprung wiesen einen spezifischen, 19%einen atypishcen Reflex auf. Die meisten atypischen Reflexe wurden als Artefakte durch die Balloninflation angesehen. Bei diesen Patienten wurde der Reflex aufgehoben, wenn die Untersuchung mittels Elektrostimulation oder Stimulation mit kaltem Wasser erfolgte, Verfahren, bei denen es zu keiner Dilatation des Rektums kommt. Außerdem stimmten diese atypischen reflexe nicht mit den Kriterien des normalen rektoanalen Erschlaffungs-Reflexes der Japan Study Group of Pediatric Intestinal Manometry überein. Die Verwendung von Elektrostimulation, kaltem Wasser oder von i.v. verabreichtem Prostaglandin F2α erhöht die Zuverlässigkeit der konventionellen anorektalen Manometrie. Eine klare und Exakte Definition des normalen Reflexes muß den atypischen Reflex ausschließen.
Résumé
48 enfants atteints de maladie de Hirschsprung et 61 enfants sains ont été soumis à une manométrie anorectale. La pression de repos dans le rectum et le canal ana ne présentait pas de différence significative dans les deux groups. La fréquence des contractions rythmiques du canal anal était significativement moins grande chez les enfants atteints de la maladie de Hirschsprung que chez les autres mais les deux groups présentaient malgré tout des recoupements importants. C’est pourquoi la fréquence des contractions ne saurait constituer un paramèter fiable pour identifier les partients atteints de la maladie de Hirschsprung.
La manométrie conventionnelle montra un réflexe de relâchement recto-anal spécifique chez 90% des enfants sains, ce pourcentage atteignant même 98%, compte tenu des réflexes non spécifiques. On note souvent des réflexes non spécifiques chez les nouveaux-nés, du fait peut-être d’une constriction du canal anal encore peu prononcée. Si, pendant l’examen, il est procédé à une injection intraveineuse de prostaglandine F2 alpha, les réflexes non spécifiques se transforment en réflexes spécifiques.
Dans 4% des cas d’enfants atteints de la maladie de Hirschsprung on observa un réflexe spécifique et dans 19% des cas un réflexe atypique. La plupart des réflexes atypiques furent considérés comme étant des artéfacts dus au gonflement du ballon. Dans cés derniers cas, il y a eu disparition totale du réflexe lors de l’examen avec électrostimulation ou stimulation à l’eau froide, ces deux procédures n’entraînant pas de dilatation du rectum, D’autre part, ces réflexes atypiques me correspondaient pas aux critères du réflexe de relâchement recto-anal normal établis par le Japan Study Group of Pediatric Intestinal Manometry.
L’électrostimulation, l’eau froide ou l’injection intraveineuse de prostaglandine F2 alpha augmentent la fiabilité de la monométrie anorectale conventionnlle. La définition précise du réflexe normal exige l’exclusion du réflexe atypique.
