Summary

Chronic intestinal pseudo-obstruetion is an uncommon cause of repeated obstruction in children of undetermined etiology, often leading to repeated laparotomies and early death. TPN combined with venting gastrostomy provides sufficient calories for growth and minimizes the need for Operations and/or hospitalizations. Seventeen children managed with these techniques experienced a sixfold decrease in the number of hospitalizations, and more than a tenfold decrease in the number of laparotomies for obstruction compared to the period before routine use of TPN and gastrostomies. Oral feedings are encouraged whenever possible.

Zusammenfassung

Die chronische intestinale Pseudoobstruktion ist eine seltene Ursache für rezidivierende Darmobstruktionen bei Kindern mit ungeklärter Ätiologie, die häufig zu wiederholten Laparotomien und frühem Tod führt. Die totale parenterale Ernährung erlaubt in Kombination mit einer Entlüftungsgastrostomie eine ausreichende Kalorienzufuhr und reduziert die Notwendigkeit einer Operation oder Hospitalisation auf ein Minimum. Im Vergleich zu der Zeit vor der Routineanwendung der totalen parenteralen Ernährung und Entlüftungsgastrostomie konnte bei 17 Kindern, die mit diesen Methoden behandelt wurden, die Zahl der Hospitalisation um das 6fache und die Zahl der Laparotomien wegen Obstruktion um mehr als das lOfache gesenkt werden. Wenn immer möglich, sollen die Patienten zusätzlich oral ernährt werden.

Résumé

La pseudo-occlusion intestinale chronique est une cause rare d’occlusions recidivantes chez les enfants. Son etiologie n’est pas determinee avec certitude, eile exige des laparotomies repetees et cause le plus souvent le deces precoce. Une alimentation entierement parenterale, associee ä une gastrotomie de Ventilation permet d’administrer suffisamment de calories et reduit au minimum la necessite d’une Operation et d’une hospitalisation. Pour 17 enfants traites par cette technique, le nombre des hospitalisations a ete inferieur de 6 fois et celui des laparotomies pour occlusion de 10 fois, par comparaison avec ce qui se serait produit ä l’epoque oü cette alimentation entierement parenterale associee ä une gastrotomie de Ventilation n’etait pas pratiquee.

Chaque fois que c’est possible, il est souhaitable que ces patients soient aussi nourris par voie orale.
