Zusammenfassung
Ausgehend von den pharmakologischen Eigenschaften des Phenothiazinderivates Chlorpromazin und des Butyrophenonderivates Haloperidol ist in den letzten Jahrzehnten eine große Anzahl unterschiedlicher Neuroleptika entwickelt und für die Therapie zur Verfügung gestellt worden. Ihre Klassifikationen nach chemischer Grundstruktur hat nie überzeugt, so daß sie auf der Basis klinischer Eigenschaften in hoch-, mittel- und niederpotente Substanzen unterteilt werden, was sich in etwa auch in den Chlorpromazin-Äquivalenten widerspiegelt, der Menge Chlorpromazin in mg, die einem mg des jeweiligen Neuroleptikums wirkungsäquivalent ist. Beide klinische Klassifikationen implizieren, daß es im Prinzip nie gelungen ist, unterschiedliche antipsychotische Wirksamkeit für die vielen heute zur Verfügung stehenden klassischen Neuroleptika zu belegen, sondern daß eine gleichwertige antipsychotische Wirkung mit allen diesen Substanzen erreicht werden kann, allerdings bei sehr unterschiedlichen Milligramm-Dosen. Diese Gleichwertigkeit der klassischen Neuroleptika im Hinblick auf ihre therapeutische Wirkung kann auf pharmakologischer Seite dadurch erklärt werden, daß alle Neuroleptika über eine Blockade von Dopamin-D2-Rezeptoren ihre antipsychotische Wirksamkeit entfalten (Richelson 1984).
