Abstract

In the last two decades, major progress has been accomplished regarding diagnosis of nature, topography, and mechanisms of stroke in the living patient by means of structural neuroimaging (i.e., CT and MRI) and ultrasound techniques (i.e., cervical, cardiac, trans-cranial). However, mapping of the extent and severity of the ischemic process itself has lagged behind, and this may partly account for the comparatively limited progress accomplished in the management of the acute stroke patient. Indeed, it is likely that the lack of established therapy for stroke despite successful methods for infarct size reduction in the experimental animal is due to including in clinical trials widely heterogeneous patients in terms of brain hemodynamic and neuronal status.
