Zusammenfassung
Intrazerebrale Blutungen machen etwa 10–15 % aller Schlaganfälle aus, haben aber einen ungleich höheren Anteil an der Gesamtmorbidität und -mortalität. In der Akutphase stellen vor allem Vigilanzminderung, erhöhter intrazerebraler Druck, Gerinnungshemmer in der Vormedikation und Hämatomexpansion klinische Herausforderungen dar, die eine sekundäre Verschlechterung der Patienten bedingen können. Ob eine rasche Blutdrucksenkung in der Akutsituation einer ICB zu einer Verbesserung des klinischen Ergebnisses führt, wurde intensiv in Studien untersucht. Anhand der Ergebnisse von INTERACT2 und ATACH-II wird der aktuelle Studienstand zusammengefasst, und im Kontext internationaler Leitlinien wird ein Fazit für die klinische Praxis gezogen. Das Ziel hierbei ist es, nicht nur Behandlungsalgorithmen aufzuzeigen, sondern auch Hintergrundwissen über die Entstehung der aktuellen Therapieempfehlungen zu vermitteln.
