Zusammenfassung
Ein fundiertes Grundverständnis zur Wirkung der Stimulationstechnik auf physiologische Strukturen ist für die effektive und sichere Anwendung der Funktionellen Elektrostimulation von großer Bedeutung. Das Kapitel beschäftigt sich praxisorientiert mit Grundlagen der Elektrodenauswahl und -anordnung und der Gestaltung der elektrischen Parameter, um Aktionspotenziale auf Nerven- oder Muskelfasern möglichst gezielt auszulösen. Es werden Möglichkeiten und Grenzen des Zusammenspiels des erzeugten elektrischen Feldes mit den physiologischen Feinstrukturen betrachtet. Speziell wird auf die Aktivierung von afferenten und efferenten Neuronen, motorischen Einheiten und denervierten Muskelfasern eingegangen, auf für Therapieplanung und -begleitung nützliche stimulationsbasierende Testoptionen und schließlich Parameteroptimierung für Therapie und Training. Der letzte Abschnitt ist den erst seit Kurzem verfügbaren neuen Möglichkeiten im Umgang mit denervierter und teildenervierter Muskulatur gewidmet.
