Zusammenfassung
Für diese sog. funktionellen gastrointestinalen Beschwerden, die durch komplexe Interaktionen zwischen Gastrointestinaltrakt und ZNS verursacht werden, gibt es keine pathophysiologischen Marker und daher keine diagnostischen Tests. Gastroenterologen finden keine pathologischen Organveränderungen. Auch für das Kindesalter sind Kriterien für funktionelle gastrointestinale Störungen neu erarbeitet worden (sog. Rom-IV-Kriterien), zu denen das Reizdarmsyndrom und andere funktionelle Bauchschmerzen gehören: rekurrierende Beschwerden über 2–3 Monate Dauer, die sich auf Defäkation bessern und mit Stuhlunregelmäßigkeiten hinsichtlich Frequenz und Konsistenz verbunden sind. Öfter als bei organisch fassbaren Krankheiten finden sich bei diesen Menschen Tenesmen, Schleimabgang, das Gefühl inkompletter Entleerung, Blähungen und Flatulenz, gelegentlich auch Sodbrennen, Dysphagie und biliäre Dyskinesie. Klagen über Kopfschmerzen, Schwindel, Übelkeit und Abgeschlagenheit oder gar Migräne können hinzukommen. Psychologisch gesehen gibt es oft Ängstlichkeit, Depression, Perfektionismus oder auch Lernschwierigkeiten. In der psychiatrischen Literatur wird auch von somatoformer Störung gesprochen. Meist sind weitere Familienangehörige von solcher Symptomatik betroffen. Die rezidivierenden Bauchschmerzen treten typischerweise nur tagsüber und periumbilikal auf, manchmal steht eine Obstipation im Vordergrund. Betroffene ältere Säuglinge und Kleinkinder präsentieren sich dagegen mit unspezifischen Durchfällen („toddler’s diarrhea“ oder „peas and carots syndrome“).
